Wegovy headaches: why they happen, the low blood sugar link and when to worry

Headache is one of the seven very common Wegovy side effects, and unlike some other weight loss injections it is listed in its own right rather than only as a warning sign of something else. Most Wegovy headaches are not caused by the molecule directly: they come from eating less, drinking less, and the changes in blood sugar and sleep that go with starting treatment. That is good news, because those causes respond to simple changes. This guide covers the causes, what helps, the low blood sugar signs to know, and the rare headaches that need urgent assessment.

Key facts

  • Headache is very common on Wegovy: more than 1 in 10 people. It was reported more often on semaglutide than placebo in the trials, though the gap was smaller than for the digestive effects.
  • Dehydration is the most frequent driver. Wegovy blunts thirst, and nausea, vomiting and diarrhoea all lose fluid.
  • Low blood sugar is the second thing to rule out. Headache is one of the leaflet's listed warning signs of hypoglycaemia, which is common in people with diabetes and uncommon in people without.
  • Caffeine withdrawal, poor sleep and skipping meals for long stretches account for many of the rest.
  • Headaches usually settle within the first weeks of a dose step as intake and hydration find a new normal.
  • A sudden, severe headache, or a headache with vision changes, weakness, confusion or a stiff neck, is not a Wegovy side effect to manage at home: use NHS 111 or 999.

Why Wegovy gives you a headache

Dehydration

The most common cause. You drink less because you eat less and because the medicine dampens thirst as well as hunger. Vomiting and diarrhoea, both very common early on, lose fluid directly. A headache that eases after a couple of glasses of water was a dehydration headache. The dehydration guide covers the fix in detail.

Blood sugar

Semaglutide lowers blood glucose after meals. In people without diabetes it rarely drops it to the point of true hypoglycaemia (listed as uncommon), but long gaps without food on a suppressed appetite can leave you running low, and headache is one of the warning signs. In people with diabetes who also take insulin or a sulfonylurea, hypoglycaemia is common and the doses of those medicines may need reducing before starting Wegovy.

Caffeine

Many people cut coffee and tea without meaning to, because they are eating and drinking less overall or because coffee makes nausea worse. Caffeine withdrawal headaches arrive within a day or two and last a few days.

Sleep and stress

Reflux, hunger changes and adjusting to a new routine can disturb sleep in the first weeks, and poor sleep is a reliable headache trigger.

Blood pressure changes

Weight loss lowers blood pressure. If you take blood pressure tablets, you can become over-treated as the weight comes off, which causes dizziness and headache. Your GP may need to review the dose.

What helps

  • Drink to a routine, not to thirst. Pale urine is the check.
  • Do not go all day without eating. Small regular meals with some protein keep blood sugar steady even when appetite is low.
  • Keep your usual caffeine or taper it over a week or two rather than stopping dead.
  • Simple painkillers such as paracetamol are usually fine. Ask a pharmacist if you take other medicines. Avoid using painkillers on more than a couple of days a week, since frequent use causes its own headaches.
  • Sleep: stay upright for a couple of hours after your last meal to reduce reflux, and keep a regular bedtime.
  • If you take blood pressure or diabetes medicines, ask your GP whether the doses still fit as you lose weight.

Low blood sugar: the signs from the leaflet

The leaflet lists these warning signs, which can come on suddenly: cold sweat, cool pale skin, headache, fast heartbeat, feeling sick or very hungry, changes in vision, feeling sleepy or weak, feeling nervous, anxious or confused, difficulty concentrating, and shaking. If you have diabetes, follow the hypo plan agreed with your diabetes team. If you do not have diabetes and get this cluster of symptoms, eat something containing sugar, then something more substantial, and tell your prescriber. Someone who is drowsy, confused or not responding normally needs 999.

The leaflet also says not to drive or use machines if you get any signs of low blood sugar, and that some people feel dizzy mainly in the first three months of treatment.

Headaches that are not a side effect

Call 999 for a headache that comes on suddenly and severely, a headache with weakness or numbness on one side, slurred speech, confusion, a seizure, or a headache after a head injury. These are not Wegovy side effects.
NHS 111 today for a headache with a stiff neck, fever and a rash; with sudden loss of vision or eyesight getting worse (which on semaglutide could be the very rare eye condition NAION, or in people with diabetes a change in diabetic eye disease); a headache that is the worst you have had; or a headache that wakes you or is worse on lying down or coughing.
See a GP for headaches that are frequent, worsening over weeks, or not responding to the measures above, and for any new headache pattern if you are over 50.

Frequently asked questions

Does Wegovy cause headaches?
Headache is listed as a very common side effect, affecting more than 1 in 10 people. Most are driven by dehydration, low food intake, caffeine withdrawal and poor sleep rather than the medicine acting on the brain directly, so they respond to simple changes.
How long do Wegovy headaches last?
Usually days to a few weeks after starting or stepping up, easing as fluid and food intake settle. A headache that persists beyond a step, or keeps recurring, is worth a GP review.
Can I take paracetamol or ibuprofen with Wegovy?
Paracetamol is usually fine. Ibuprofen and other anti-inflammatories can irritate a stomach that is already prone to reflux and gastritis on Wegovy, so ask a pharmacist first. Avoid regular painkiller use on more than two or three days a week.
Is a headache a sign of low blood sugar on Wegovy?
It can be. The leaflet lists headache among the warning signs of hypoglycaemia. It is common in people with diabetes on Wegovy, especially with insulin or a sulfonylurea, and uncommon in people without diabetes. Other signs together, such as sweating, shaking, hunger and a fast heartbeat, make low blood sugar more likely.
Are migraines worse on Wegovy?
Migraine is not listed separately in the leaflet. Dehydration, skipped meals and disrupted sleep are classic migraine triggers, so people with migraine may see more attacks early on until intake and routine settle. The same measures help.
